APPLICATION WAIVER

In exchange for the consideration of my application for employment by Learning Point Group, LLC. (hereafter called "Company") I agree to the following (PLEASE CHECK ALL POINTS TO WHICH YOU AGREE): *

1) Neither the acceptance of this application nor the subsequent entry into any type of employment relationship, either in the position for which I am applying or any other position, and regardless of the contents of employee handbook, policies, manuals, or other documents, and the like as they may exist from time to time, or other Company practices, shall serve to create an actual or implied contract of employment, or confer any right to remain an employee of Company, or otherwise to change in any respect the "employment-at-will" relationship between Company and myself, and that relationship cannot be altered except by a written instrument signed by the President of Company. Both myself and Company may end the employment relationship at any time, without specified notice of reason.
2) If employed, I understand that Company may unilaterally change or revise Company's benefits, policies, and procedures, and such changes may include a reduction in benefits or wages.
3) I hereby authorize Company to investigate and verify any and all statements and information contained in this application. I hereby understand that the misrepresentation or omission of facts called for is cause for dismissal at any time without prior notice. I hereby give Company permission to contact schools, previous employers, (unless otherwise indicated herein), references, and others, and hereby release Company from any liability as a result of such contact.
4) I understand that (a) Company has a drug and alcohol policy that provides for preemployment testing as well as testing during employment; (b) consent to and compliance with such policy is a condition of my employment; and (c) continued employment is based on the successful passing of testing under such policy. I further acknowledge that continued employment may be based on the successful passing of job-related testing and examinations.
5) I acknowledge that, in connection with routine processing of my employment application, Company may request a background check to further determine my character, credibility, and suitability for employment.
6) Finally, I acknowledge that the signing of this application does not constitute a job offer. If Company decides to hire me, a written job offer will be provided. In the event I am hired, my initial employment with Company shall be probationary for a period at up to sixty (60) days.
